The Application Process

Initial contact to Moving On Head Office is made by the Social Worker or Probation Officer of the Young Person

• An information pack and application form about Moving On will be sent.
• The Social Worker/Probation Officer should complete the application form together with the Young Person. Once completed the forms should be returned to the Moving On Head Office.
• If the Young Person fulfils Moving On criteria, a meeting will be arranged with the Young Person, Social Worker/Probation Officer and Moving On Project Leader.
• After the meeting the Young Person decides whether the Moving On project can meet their needs.
• If the Young Person agrees to the programme, a contract for service agreement will be sent out.
  No work by Moving On will commence until the agreements are completed in full.
• The appropriate outreach worker will then meet with the Young Person, who will be required to complete a Young Person’s contract and fill in a questionnaire. A comprehensive plan will then be drawn up in order to place the young person.
