Summary of Services
Each Young Person is allocated an experienced outreach worker. The outreach worker is selected according to the needs of the Young Person in order to provide a comprehensive care package.
Once the placement commences, the comprehensive care package will be drawn up between the Young Person and the outreach worker. The package will be drawn up in accordance with the Young Person’s needs and circumstances. Areas that are covered in the package will not just be limited to living skills, (cooking and budgeting), but include; health, safety and hygiene (both personal and home), social skills (networking, communication, relationships), emotional issues (anger management, coping with loneliness, etc).
Each young person will receive help and support in areas of training, employment and education, such as guidance filling in application forms, compiling a CV, interview techniques, assessment of requirements, skills and capabilities, both educationally and vocationally.
Each comprehensive care package is reviewed and evaluated on a regular basis, this ensures that the progress of the Young Person is monitored and that the package is meeting individual need. Monthly updates are filed so the progress is monitored. The Young Person may have access to their updates and also their files, which are held by Moving On.
On full and satisfactory completion of the placement, each Young Person is entitled to receive a Moving On leaving grant. This is the equivalent to the sum of £10 per week of successful placement, (e.g. if the Young Person is placed successfully for 2 years they would be eligible for a leaving grant of £1040, for a placement of one year £520 etc). Should there be any damages and/or losses to the Young Person’s accommodation during the placement, then the costs of these will be deducted when calculating the final Moving On leaving grant.
Each Young Person involved in education or training will be provided with a weekly bus pass or travel card to enable them to travel to all necessary appointments. A weekly phone card will also be provided.
Where necessary Moving On will provide the Young Person with transport to move their belongings to any accommodation that is provided by us.
Moving On continually update their database of local landlords and visit on a regular basis to ensure that the standard of accommodation offered is satisfactory.
Each Young Person on the Moving On program is placed in a private sector one-bedroom flat, studio or bed-sit, whenever possible this will be within proximity to the area requested by the Young Person or referring agency. Rent is normally exclusive of gas and electricity and the Young Person will work along with their outreach worker on budgeting for these essential services.
Due to the independent nature of the Moving On program, it is expected that the Young Person will have reached a certain level of independence already, e.g. we would expect that the Young Person to have basic cooking skills and living skills. Each Young Person accepted onto any of our services must be equipped with the following essential items:
- Adequate clothing, for leisure and employment
- Bedding, quilt, pillow, covers and sheets, towels
- Pots, pans, crockery and cutlery etc.
Moving On Care Management Ltd, will provide each Young Person with a welcome starter pack on arrival into accommodation, this will include basic provisions and household items. Each Young Person will also receive a Birthday gift and Christmas hamper.
